PrimeCareHealth Michelini Center for Health is located in the Medical Office Building of AMITA Health Saint Elizabeth Medical Center, located on Western Avenue between Division Street and North Avenue.

By Train: Blue Line to Damen or to Western

Free parking is available in the Visitor’s Lot. Remember to get your ticket validated at the security station on the first floor before leaving.
