Dear PrimeCareHealth Patients,
During the COVID crisis, we want you to know that a doctor is available 24 hours a day, 7 days a week to assist you with any needs. After 5pm during the week and throughout weekends, when you call your PrimeCareHealth center, our staff will answer the phone, ask the reason for your call, and write down your contact information. One of our on-call doctors will call you back for a telephone visit.
Please don’t hesitate to contact us with any needs that arise outside of our normal operating hours. We only ask that you do not use the after-hours system to schedule routine daytime appointments. We are here to support you and your family during these difficult times.
To best accommodate our patients impacted by COVID, we are adjusting non-urgent scheduled appointments and procedures beginning Monday, March 23rd. If you feel sick please call your medical site and speak to a staff member who can assist you accordingly.

PrimeCareHealth has eight neighborhood locations throughout Chicago: six primary care centers, two dental centers, and two medication-assisted treatment centers. PrimeCareHealth is a Federally Qualified Health Center (FQHC) providing quality, affordable health care to medically underserved populations.
Everyone is welcome
Your medical home is where you and your family can go for regular checkups or when you’re sick. We provide medical, behavioral health, and oral health care for people of all ages, regardless of ability to pay. Your medical home is also the place to come for referrals, get help from a case manager, or sign up for insurance. For your convenience, we offer appointments during the day, evenings, and on the weekend. You can also communicate with your provider via the patient portal for easy access.
At your medical home, you have a care team to ensure all your needs are met to the highest quality. Your care team includes: your primary care provider (your doctor, nurse practitioner or physician assistant), medical assistant, and nurse care manager. Your care team may also include a behavioral health provider and a case manager.
Your team will provide you and your family with care that is evidence-based and follows scientifically proven guidelines. You will be offered routine screening tests and other health prevention services as recommended by the United States Preventative Health Services Task Force (USPSTF), the foremost group in the U.S. for advising medical professionals in providing preventative care.
