This morning, PrimeCare received a call from the White House Office of Personnel Management, Katherine Archuleta, to congratulate us on our outreach and enrollment work, particularly to immigrant populations. Speaking with our Manager of Outreach and Enrollment, Graciela Guzman, Ms. Archuleta remarked that the White House is very grateful for all of our efforts on behalf of our Hispanic/Latino and Polish immigrant patients and that our work is a model for this kind of outreach. PrimeCare’s efforts came to the White House’s attention through work with the national organization Enroll America and our partners in our Federal Navigator grant, including Erie Family Health, the Polish American Association, the Logan Square Neighborhood Association, CommunityHealth, Norwegian American Hospital and Near North Health Services Corporation.
Last weekend, this partnership helped 600 uninsured individuals find the policy that was right for them. Next weekend is the last chance to enroll, so be sure to come to our awesome enrollment event:
Presence Saint Mary Hospital, 2233 W. Division Street, 3rd floor. They’ll open at 8am on Saturday and remain open until the last applicant is enrolled!
Erie Family Health, 2418 W. Division — 8-4 on Saturday and 8 till last applicant enrolls on Sunday

Puerto Rican Cultural Center, 2700 W. Haddon — 8 am to 10 pm Saturday, 8 am to 11 pm Sunday

Association House, 1116 N. Kedzie — 8 am to 10 pm Saturday, 8 am to midnight on Sunday

- Photo ID
- Social Security Card
- Proof of Income (Paystubs for last 30 days, tax return)
- Immigration documents (Permanent Residency Card, Certificate of Naturalization, Visa, employment authorization)
- Proof of Address (Utility bill)
