Training Physicians for the Future

As a family medicine practice, PrimeCareHealth is committed to providing health services for life across all of the communities we serve. To ensure that future generations of doctors share this commitment, our West Town and Wicker Park centers serve as teaching and practice sites for the AMITA/PSMEMC Family Medicine Residency Program. They provide doctors enrolled in an accredited three-year Family Medicine Residency with hands-on experience caring for a diverse patient population. Overseeing their work are twelve board-certified family practitioners who also serve as faculty physicians. Our medical residents provide the full range of medical services offered at every PrimeCareHealth center.

In addition, PrimeCareHealth provides behavioral health training and experience to externs from some of Chicago’s most highly regarded masters and doctoral level psychology programs. During their year at PrimeCareHealth, they perform behavioral health assessments, provide individual and group therapy, and support medical staff in dealing with patients with depression and other disorders.

PrimeCareHealth exceeded national centeral quality benchmarks, including Healthy People 2020 goals, in chronic disease management, preventive care, and perinatal/prenatal care. In 2017, we were named a Health Center Quality Leader, ranking in the top 30% of health centers in the country.


Additionally, we were granted the Enhancing Access to Care Award for increasing the total number of patients that received comprehensive quality care and the Clinical Quality Improvers Award for having a 10% improvement in multiple centeral quality measures.

Patient Centered Medical Home

Your medical home is where you and your family can go for regular checkups or when you’re sick. We provide medical, behavioral health, and oral health care for people of all ages, regardless of ability to pay. Your medical home is also the place to come for referrals, get help from a case manager, or sign up for insurance. For your convenience, we offer appointments during the day, evenings, and on the weekend.  You can also communicate with your provider via the patient portal for easy access.

At your medical home, you have a care team to ensure all your needs are met to the highest quality.  Your care team includes: your primary care provider (your doctor, nurse practitioner or physician assistant), medical assistant, and nurse care manager. Your care team may also include a behavioral health provider and a case manager.

Your team will provide you and your family with care that is evidence-based and follows scientifically proven guidelines. You will be offered routine screening tests and other health prevention services as recommended by the United States Preventative Health Services Task Force (USPSTF), the foremost group in the U.S. for advising medical professionals in providing preventative care.
